Referring clinicians check eligibility in a lot of different places — criteria PDFs, proformas, intranet pages, e-RS entries. QGenome needs to be visible in all of them.
Where to add QGenome
The PDF referral criteria document — make sure every version on every website is updated
The referral proforma or Family History Questionnaire (FHQ).
The e-RS Directory of Services entry for the Genetics service (referrer-visible description field).
Any shared ICB / integrated care board intranet pages aimed at primary care.
GP education packs and Local Medical Committee (LMC) newsletters.
Update your required attachments checklist
Add a QGenome report to your standard list of required attachments so referrers know it's expected before they submit.
For example:
Required attachments
- Completed Family History Questionnaire (FHQ)
- QGenome report (generated via the Clinibee app — www.qgenome.co.uk)
- Any relevant histology or imaging
- Patient consent to be contacted (if applicable)
